If you're a new client I will need your physical address. I have a packet of information I will send to you prior to our first meeting. Much of the information is copyrighted and can not be sent via email. The surveys, office policies, history and symptom questionaires will require about an hour to complete. My office address is 430 Crane Ave. Ste. 109; Turlock, Ca 95380. Depending on when your appointment is, it may be better to come to my office to get the information packet.
